Change of details

The process if you hold a Disabled Person's Freedom Pass and have recently changed your name or address

If you have an Older Person's Freedom Pass or a Disabled Person's Freedom Pass, you need to provide us with proof if you have changed your name or address. Please see accepted documents below. 

Proof of address

  • Current council tax bill/letter/payment book
  • Current council/housing association rent book/statement/letter/tenancy agreement
  • Residential utility bill/letter (excluding mobile phone bills) dated in the last 3 months
  • HM Revenue and Customs letter dated in the last 3 months
  • Department for Work and Pensions letter dated in the last 3 months
  • Occupational pension letter dated in the last 3 months

Proof of name

  • Current passport
  • Medical card
  • Current driving licence
  • European ID card

Please provide a copy of these documents to us via email or post.

You will also need to provide your previous details and your date of birth so that we can locate your record.

Please note: Failure to inform us of a change of address will result in your pass being cancelled if you do not respond to a letter that is sent to your old address. If your pass is cancelled and you have not informed us of your change of address you will have to pay for your travel until you contact us and you receive your new pass. Any travel costs you incur are non-refundable.
